I voluntarily choose to receive services from Danielle Gerkens, which may include Reiki, energy healing, intuitive healing, intuitive guidance, psychic-mediumship readings, distance sessions, space clearing, group experiences, or related wellness and spiritual practices.

I understand that these services are complementary practices and are not medical treatment, psychotherapy, psychological treatment, or another form of licensed healthcare.

I understand that Danielle Gerkens does not diagnose, treat, cure, or prevent medical or psychological conditions and does not prescribe medication or provide medical, psychological, legal, or financial advice.

I understand that these services are not intended to replace appropriate care from licensed medical, mental health, or other qualified professionals.

I understand that Reiki and energy healing are complementary practices and that individual experiences may vary.

I understand that no specific physical, emotional, mental, spiritual, or other result is guaranteed.

For in-person Reiki or energy healing sessions, I understand that the session may involve light, non-invasive touch or hands positioned near the body.

I understand that I may request that no physical contact be used, may establish or change boundaries regarding touch at any time, and may ask for any portion of a session to stop.

My continued participation is voluntary.

I understand that intuitive guidance and psychic-mediumship readings are subjective in nature.

I acknowledge that no intuitive impression, prediction, message, communication, or other information received during a session can be guaranteed as accurate or independently verified.

I agree to use my own judgment regarding any information shared during a session and understand that I remain responsible for my decisions and actions.

I acknowledge that I am responsible for determining whether participation in these services is appropriate for me.

I agree to communicate relevant boundaries, concerns, discomfort, or changes in my ability or willingness to participate.

I understand that I may ask questions, decline an exercise or form of touch, request a modification, or end my participation at any time.

I remain responsible for seeking appropriate professional medical or mental health care when necessary.

I understand that wellness, energy, intuitive, and reflective practices may sometimes bring awareness to emotions, memories, physical sensations, or personal experiences.

I understand that I am not required to disclose personal or traumatic information in order to receive services.

I may pause or discontinue participation at any time.

I understand that Danielle Gerkens treats information shared during private sessions with care and discretion.

I acknowledge, however, that these services are not psychotherapy or medical treatment and that communications may not carry the same legal privileges that can apply to certain relationships with licensed healthcare or mental health professionals.

I understand that participation is voluntary and acknowledge that individual responses to complementary wellness practices vary.

To the extent permitted by applicable law, I voluntarily assume the ordinary and reasonably foreseeable risks associated with my participation in the services.

To the fullest extent permitted by applicable law, I release and hold harmless Danielle Gerkens and Danielle Gerkens, LLC, as applicable, from claims arising from the ordinary and inherent risks of my voluntary participation in the services, except to the extent a claim cannot legally be waived or released.

Nothing in this agreement is intended to waive rights or liability that cannot lawfully be waived under California law.

I acknowledge that I have reviewed and agree to the applicable booking, payment, cancellation, rescheduling, late-arrival, and refund policies presented at the time of booking.

I also acknowledge that I have had access to the Client Disclaimer and Terms & Conditions.

By electronically accepting this agreement, I confirm that:

• I have read and understand this Client Consent + Liability Waiver.

• I understand the nature and limitations of the services being offered.

• I understand that participation is voluntary.

• I have had the opportunity to ask questions before participating.

• I understand that I may withdraw my consent to participate at any time.

• I am at least 18 years of age or have completed any separate consent process required for a minor.

• I voluntarily consent to receiving the service I have booked.
